New York State is divided into 58 local departments of social services (LDSSs). The five boroughs of New York City comprise one of them. Each of the others corresponds to one of the 57 counties in the rest of the state. LDSSs provide or administer the full range of publicly funded social services and cash assistance programs.

If your Medicaid is with your LDSS, to order a new Medicaid Benefit Identification Card, please call or visit your local department of social services. If your Medicaid is with the Marketplace (NY State of Health) and you need to order a new benefit card please call the call center at 1–855–355–5777.
